What Is Forward Head Posture?

Forward Head Posture is a postural pattern in which the head rests in front of the shoulders instead of staying aligned over the upper body. This position changes the load placed on the neck and upper back during standing, sitting, walking, or screen use. The pattern may appear with rounded shoulders or increased upper-back curvature.

  • It is not the same as a temporary head position.

How Forward Head Posture Works

Forward Head Posture describes the relationship between the head, neck, shoulders, and upper back. A clinician observes how the head rests over the body during relaxed standing and may compare that position with movement during sitting or walking. The assessment focuses on alignment, muscle activity, joint motion, and reported symptoms.

The pattern often develops through repeated positions, such as looking down at a phone or reaching toward a computer. Neck muscles may work continuously to support the head, while nearby shoulder and upper-back muscles adapt to the altered position. Cervical Spine movement and thoracic spine posture are considered together because one region can influence how another region moves.

From the Arrowhead Clinic Chiropractor McDonough team

When we assess Forward Head Posture during Neck Pain Treatment, we observe the head, shoulders, and upper back in more than one position. We also consider neck movement and the person’s reported symptoms. Our team uses these findings to guide the clinical discussion and distinguish a posture pattern from other possible contributors to neck discomfort.

Forward Head Posture in Practice: A Real-World Example

A person who spends much of the day at a computer may notice the head drifting ahead of the shoulders by afternoon. During a neck pain visit at Arrowhead Clinic Chiropractor McDonough, the posture is considered alongside movement and symptom patterns.
